What would be the underlying cause? Have they permanently damaged themselves?

some context would help. Has that person tried to have sex and failed?

No sexual activity whatsoever and still in PAWs, no erections, no semi's, no arousal, flatline, after like 14 months.

Sexual activity could possibly jumpstart libido. Also, hormonal imbalances and poor blood flow can play a role. Make sure diet is good and promotes testosterone production. Exercise is important for blood flow and overall well-being.

I see. Are fried dopamine receptors reversal or once you desensitize badly enough, they can't recover? I'm scared...

I think they can recover. It might just take more time. And try to stay away from other dopamine stimulating activities like video games and social media scrolling.

So if I play video games, I can't recover from PIED at all? That makes no sense. How can video games stop dopamine receptor growth, especially if the content I enjoy has no sexual stuff?

I understand where you're coming from because video games do affect the dopamine receptors, but I do not look at video games or play them to get off for sexual gratification.

You can, but it certainly won’t speed up your recovery.

Does music hinder your recovery too? I don't get it, why torture yourself so much?

It's possible it's not PIED. It could be something physical. I thought I had PIED but it turns out I have really low testosterone. See a doctor.

No, my issue is PIED. I had my thyroid checked and had a physical recently. Everything came back normal.
