We have all experienced it. I will try to describe it, and I hope we can understand it more through this discussion and talk about ways to combat it.

I have been able to identify some of my main PMO cues: frustration, boredom, loneliness, the passing of my average no-PMO number of days, raw sexual triggers, etc.
These cues or triggers kick in the response my brain has gotten used to: PMO - my brain thinks about PMO automatically whenever one of these cues are activated for a certain threshold.

The cue activating the PMO habit though doesn't mean that it is the end of the world, because at that stage I am still sane and can decide to shrug it off. But here's the trouble: after that initial activation, even if I ignore the urge for extended time (hours to even a day or two) my brain still readily kicks in the habit whenever a cue at a much lower threshold appears. That is when my brain gets really foggy - it cannot get it out of my head anymore, and it always ends with a relapse.

What are the causes of this brain fog? How can we combat it?

Luckily for you(;)!) i have read a lot about brain fog because i struggle a lot with it and let me tell you what i know
First it is caused because our dopamine receptors are exhausted and when dopamine is discharged(not sure if i used the right word) in the brain, our dopamine receptors are so exhausted that they don't fully receipt dopamine and dopamine is responsible for a lot of things such as motivation,focus,mood,decision-making,creativity,even regulating sleep and cognition.
which is why when our dopamine receptors are exhausted and don't receive dopamine well.we lack all the above abilities to some degree.I also read that brain-fog is generally caused because neurons can't connect to each other well and send signals.
So that was the cause of brain-fog since you wanted to know. But what matters is how we can fight it until it goes away in the second week of no pmo.(that is actually experimented and proven that it takes at most 14 days for dopamine receptors to heal)

You can help yourself get better by a few tasks.
1-Exercise,aerobic exercise to be specific.jogging,swiming,hiking,... not everyday though.Every other day is good enough for like 30 mins.
2-get enough sleep, not too much sleep but enough. At most 8 hours in 24 hours of a day
3-also lessen sugar intake, they actually stimulate the brain in a very similar way pmo or drug use does.
But you can't rush it,just wait for 2 weeks and it will go away, what matters is that we stop pmo.

There is one thing i read a few days ago that is working well for me when dealing with brain-fog.I read it in a book."The Power of Now" it doesn't matter what the book is all about it suggests a way to clean your mind from all useless thoughts and be more focused.it has worked for me.Google it, you can find the pdf.Just read the first chapter, it's only 15 pages.Basically you become an observer of your mind and the thoughts that pass in your mind and when you become an observer you can neglect the thoughts in your mind which are mostly negative and useless.I think it's kind of like what people who meditate do.clearing their mind of any useless thought.
I know it might sound to easy or weird or stupid to work but read it and try it.It works well for me these days when i have brain-fog and can't easily study.I relax for a moment and stop all the useless thoughts and then just focus on the task at hand.

My biggest problem was brain fog. After I stopped PMO, I started to exercise just 10 - 20 mins a day. It doesn't matter what exercise you do, the goal is to increase your heart rate.
I am not exaggerating, my performance in class has improved so much that I can't measure it. I couldn't sit and listen for more than 5 mins. My brain would literally stop working and my head would start to hurt and stupid thoughts about porn would started to trouble me. All of that hasn't faded completely yet because its just been a month so far, but, my concentration has increased over 20 times.

In the first place, work against these cues by doing useful fulfilling activity.
Boredom and lonelineness can obviously very easily be conquered by some activity!

Frustration can also be managed by increasing your patience.

Maybe you have only focussed on abstinence, but you have not built up new habits, as I mentioned.

Your brain wants a rush of dopamine. It is searching in its memory on how to get it, and there it finds only one single entry: Fapping. And so it is trying to activate that one and only memory part. You try to block that, and that causes the fog. But this is a battle you cannot win.

You can change that in the first place by adding additional entries to your "database". Show your brain alternative ways to get the dopamine. And then it can choose. And to be able to choose is called freedom.
