Defining your problem is the first step in overcoming it. You know you have a problem, and you're admitting it openly; that's the first step to recovery. Start a journal, find an AP if you need to, and get to work! The next time you feel urged to do that....fight it. Don't give in! Success breeds success!

Start a new chapter starting now. Install web filters. Do everything you can to get 1 day clean. Then 2 days. Your whole body will be screaming for P, but you are tough, you can break this. On the other side is self respect, happiness, the chance to love yourself and be good enough to find love from an actual woman one day.
