I think it's really good that you want to change and in some senses already makes a huge difference. At the same time I think you need to be specific about it. Have you tried journaling? Essential what you could do in there is write down exactly what you want to change, how you plan to go about it and what impact you think it'll have on you.
I know for me personally, it helps to have it recorded because then I'm thinking deeply about it and can look back whenever I'm slipping back into bad habits or to see how far I am from those bad habits.
