I am 33 yrs old. Always had good erections and could keep them as long as I wanted to. Last hookup in Fall 2019, had sex three times within few hours.

Then fast forward a year, I suffered from a burnout and was depressed. Met a girl, could get it up whilst kissing but LOST ERECTION IMMEDIATELY when I stopped and noticed the same thing for masturbation. I was puzzled...

Same week, doctor gave me pills. When I took 10g (one pill) I could go 3x easily a night.

That was in December 2020. I then lost some weight, ate healthier stuff and exercised. Result: started to get more often morning wood (at 80%, can keep it whilst.laying for about 2 to 3 minutes...but will lose it when I go to the loo).

And 3 weeks ago, I was super aroused for some reason, went to my girl and was able to sleep 3x with her WITHOUT pills. However, a few days later, we met again and after the first round sex I could briefly get hard for round 2, but lost it before penetration. Took half a pill (5mg) and was able to sleep with her several times.

My question to you guys is: given my situation, what do you think, I am suffering from VL in?

I am especially worried about what @Polymoly said: I fear that the fact I can best maintain erections with pills is because the increased blood flow compensates for the higher blood outflow due to VL.

Any views on my situation? Haven't done a Doppler so far.

 

Ok so now I suggest you to see a good doctor and explain him what's going on, and eventually he will do a doppler, then try to not take pills for a good amount of time to see what happens in both sex and solo masturbation and you also need to know for how long the symptoms will remain the same

I should note that I am almost never really in the mood as compared to earlier times. I attribute this extremely low libido definitely to the burnout.

I just started a few days ago daily 5mg Tadalafil before going to bed. With that sex us possible for me twice a day, even with low libido.

 

You can do blood tests to see if there's some issue with your libido,
if there are no external psychological factors or hormonal problems causing your libido to drop there's a possibility that your ED situation is conditioning your libido since you know that you aren't performing well right now. For the first question it is possible to be consistent without VL but 90+% of cases with consistent ED still have a physical problem

The external psychological issue could be a burnout (job related massive stress and a breakup with my fiance) -- that happened just after I used to have normal erectile function 1,5 yrs. ago. Something (physical and/or psychological) must have happened in that 1,5 yrs to give me ED. During that time I also heavily masturbated, i.e. edged, for several hours every day.

What I find puzzling is that my condition has improved over the last 3 months when starting to make lifestyle changes (sports, foods and reducing stress at Work). My erections do NOT die down within seconds anymore, I can keep them for LONGER even without pills.

Only difference is that 1,5 yrs ago I could keep them for dozens of minutes while now I can only keep erections for a few minutes (with pills I can keep them as long as before). And the other difference is certainly that I kinda cease to have a desire for sex.

Blood tests regarding T levels came back at average level.

 

Actually, another question is:

If it's true that ED pills just compensate for VL with higher blood flow, how would a penile Doppler with injection be different? Isn't the idea of the injection to widen blood vessels to induce an erection? In that respect ED pills and the injection used for a Doppler could both compensate for higher blood loss due to VL, even if able to maintain an erection, right?

 

You're right, in fact the doppler is not a valid diagnostic tool to find out if you have a VL as it allows you to generally ascertain the conditions of the penile arteries, where I live, nowadays andrologists take more into consideration the patient's history than the doppler. Many people I got to know who did the doppler years ago tested negative for the doppler but still had symptoms of the disease, which never faded. In these cases, you can choose to continue further with the investigations (cavernosometry / biopsy), but few do it because the examinations are more invasive.

I really hope that things can turn out well...I mean I was able to have sex 3x within 5 hours without pills a few weeks back -- 4 months ago I would lose my hard-on within seconds. Given the progress, I do hope that it is unlikely to be a VL, but perhaps a combination of low libido due to burnout/depression and massive PMO.

 

How was the quality of the erection when you were having sex? Was it rock hard like usual?

 

You mean when I had sex a few weeks ago without pills? Round 1 was rock-hard, second and third rounds a little weaker when penetrating first, but rock-hard when switching positions.

I generally noticed (whether I use pills or not) that between foreplay, putting condom on and inserting I get a little bit softer, unless I am super aroused. But when we switch positions I do NOT get softer. Performance anxiety before starting?

When I take half a pill (5mg) I can go four or five times a night, but the last time I can really feel I am softer. Bur normally I am not super aroused then.

I hope so, too. But as I a said, this was a few weeks ago. These days I sometimes still need half a pill if I want to get rock hard for more than one round.

...which could simply reflect that I may just not be in the mood for too much sex. I don't know this whole ED thing is so much stress.
