You have to block the vision/fantasy at a mental level, before you start to fantasize. You can try to focus on something else. One thing can be your breath, as in many meditative techniques. The other one is something I read on the subreddit:

This problem is preventing you from reaching your goals and dreams. First, realize this. Now, visualize your goal mentally. Let's say you want to graduate successfully. Imagine yourself in the graduation ceremony, holding your diploma and shaking the hand of your professor. Something like this.

Now, every time you notice a trigger (fantasizing about your fetish), drop that thought and visualize your dream. This will both divert your attention and make you realize that you are making the right choice between two paths.

 

Is it possible to kill the fetish though? Like make it so you really don't have any interest anymore? Will it fade over time if you just keep blocking the fantasies? Its probably my #1 problem right now

 

For me they did fade away. I wouldn't say they are "killed", since it is not a binary thing. They become less and less, but I'm aware that if I get into a porn binge, I can watch that stuff again, and those fantasies are strengthened.
