Porn Addiction coming back

Hi

I started masturbation at the age of 16 and went on with it for many years. Midway when internet porn came up masturbatiin was aided with porn. But main addiction seems to be the pleasure of orgasm and porn seems to be only a stimulant for the same. When I became 26, I started having sever pain in my reproductive organs and couldn’t go on with masturbation anymore. In fact the pain was on the testis and the nerves connecting them with the spine on the lower back. Such a sever pain I couldn’t even walk. So if I masturbate ever I get sever pain and it takes weeks or months for the pain to even reduce. So literally became a bit sexually disabled and avoided marriage proposals during the age when I was supposed to get married or get into any relationship. No doctor could help me with this issues and the pain never went away and I fact was always there. Led to depression and suicidal thoughts. Trying to find alternate alternate techniques to heal and found meditation to be a great help. Even with the sever pain I could not be without masturbating so would eventually end up masturbating once a month atleast but still suffered the pain and depression. But meditation gave me a good relief mentally and I was able to give up PMO totally for six years in a row. Not even once it happened. But the pain was still there. After six years of meditation the body started healing and the pain started subsiding. When the body starts to heal the habits are creeping back again and attacking me again and now I am not able to control myself again like when I was young. The meditation heals the body but the habit is coming back and attacking and putting my health down. I really feel I am very close to solving this problem permanently but this attack is hindering me a lot. Please help/ suggest/guide me to over come this temptations from my past so that I can once for heal my body completely
