So, for the more academically inclined among us... I've just found this literature review on the current state of play (published this year, so fairly up to date.) I don't think it's particularly well written (the word "nowadays" should never appear in an academic article...) but it does give a useful overview of where the scientific community have got to in terms of diagnosis / treatment etc. Enjoy (if you can...)
