I'd say definitely yes. This is reinforcing the role of masturbation. It may even be worse than masturbating on your own as you are placing it above sex as a means of completion. But on the last part I don't know for sure.

I'm not sure- masturbation can be a part of sex- if your orgasm was the result of sex with a woman (I see you are allowing O on your journey) and you pulled out as you felt the sensation of orgasm and then jerked briefly to allow your climax to fully happen then I would say no it doesn't count...

However, if you had to fap for a while and imagine pornographic thoughts in your mind that didn't relate to the woman in order to orgasm then yes I'd say that counts as a relapse.
